Practice Closure Information
Our practice will be closing permanently on November 1, 2026. We encourage you to begin the process of establishing care with a new provider to ensure continuity of care. With your permission, copies of the pertinent information from your record may be made available to a provider of your choosing. If you have an upcoming appointment scheduled before the closing date, we will be happy to assist you with your transition during your visit. If not, you may choose any provider of your preference.
For routine eye care: There are numerous Optometrists in the surrounding area who can provide routine eye care, including several Walmart independent Optometrists. A new provider can be found by visiting: www.NCEyes.org/Find-an-Eye-Doctor.
For medical eye care: For patients requiring continued care for medical eye conditions (e.g glaucoma, macular degeneration, etc) we recommend contacting any of the following practices:

Patient Records Access
After the closure, patient records will be transferred to Morgan Records Management, LLC, the official records custodian. Records will be securely maintained for at least 6 years from your last date of service, in compliance with applicable laws.
To request a copy of your records:
- Before October 2023: Submit a written request directly to our office. Click here for more information.
- After November 1, 2026: Submit a records release form with Morgan Records Management at:
www.MorganRecordsManagement.com

Please note:
- A fee may apply for copying and sending records, according to state and federal laws.
- There may be a brief delay in record access during the transition of the records to Morgan Records Management.

Records Request Information

For most patients a full copy of service records is not needed.
Your next doctor most often only needs your
most recent exam/office visit results.
Due to the volume of records copy requests, in order to complete all requests before office closure, the following policy will apply:

The last day to request full records copy is October 16

The last day for most recent exam copy requests is October 23

The last day to pickup records copies is October 29

From October 24 to November 1, we will be unable to fulfill records requests as we close the office

After 11/1, please go to MORGAN RECORDS MANAGEMENT to request your records

All records requests require a written/signed request form. If we do not receive the completed form, we cannot provide you or anyone else a records copy.
If you or your next doctor needs a full record copy in the future, they will be available for 6 years (10 years for minors) after our practice closes on November 1 through Morgan Records Management (fees may apply).
If you would like a paper copy of your records prior to October 30, we will be glad to provide you with them according to the following fee schedule:

Most recent eyeglass or contact lens prescription: in person pick up or fax - no charge, mail - $10

Most recent visit: in person pick up or fax - no charge, mail - $10

Full patient record: in person pick up or fax - $35, mail - $45
Please note: with the exception of eyeglass/contact lens prescriptions and retinal photos,
record copies CANNOT be emailed.
